Facility Request Form

Club advisors, coaches, or other staff who need a space to hold meetings or events, must fill out the West Campus Facility Request Form.  Make sure to complete all the fields. The space, date, and time are subject to availability.  School sports and school events take precedence.  Outside vendors must reach out to the district Civic Permit Department by filling out the Civic Permit Form.     The Vice Principal, Ms. Roland will notify the requestor or the civic permit facilitator of the result.
